United States Plans

Flexible Spending Accounts (FSA) - (Section 125 Cafeteria Plans) that are available through your employer are designed to assist you with paying for medical expenses including eyeglasses. You can use the debit card provided by your employer to purchase eyeglasses through . If do not have a debit card, you will need to submit OptixRx's receipt for your reimbursement.

Health Savings Accounts - You can also use the debit card available from your Health Savings Account to purchase eyeglasses from . If do not have a debit card, you will need to submit OptixRx's receipt for your reimbursement.

Out Of Network Benefits -  does not currently participate with any Vision Insurance Plans. All Vision Plans have out of network benefits which are designed for the situation when you want to purchase glasses from a non-participating provider. After you have purchased your glasses from  you will need to submit OptixRx's receipt for your reimbursement. Our receipts will include the following information:

• Name of individual the service is for
• Name of provider
• Date of service.
• Description of product.
• Prescription Filled
• Amount of claim

Canadian Plans

Canadian insurance providers may pay a vision claim to the individual if you have a vision benefit on your plan. Our receipt contains the following information that is required. Please check with your provider to see if you are eligible through your health care plan.

• Name of individual the service is for
• Name of provider
• Date of service
• Description of product
• Prescription Filled
• Amount of claim
